CCOS™ is a cohort-based stabilization and workforce routing program (6–8 weeks plus aftercare). This form is used for intake, screening, enrollment, referral, and outcome tracking. Submission does not guarantee enrollment.

HHHI delivers non-clinical peer support, workforce training, and community-based programming. Clinical services are provided through licensed healthcare partners using compliant documentation, consent, and supervision protocols.
Programs are implemented in collaboration with education, healthcare, workforce, and community partners to ensure responsible, compliant service delivery.
